Management Agreement
Heart Disease, Diabetes and Asthma Prevention Programs Will Help Control Rising
Health Care Costs and Improve Employee Health
Farmington, CT (April 2004) — Cardium Health, one of the leading
providers of heart disease, diabetes, and respiratory disease management programs to Fortune
1,000 companies, announced that it has entered into a new four-year agreement with Tenneco
Automotive to provide its health promotion and disease management programs to Tenneco Automotive
employees and their dependents. The programs will focus on enhancing health and preventing
the progression of heart disease, diabetes, and respiratory disease by: reducing complications
and re-hospitalizations; minimizing disability and absenteeism; and helping to contain
the cost of direct medical claims.

Cardium Health’s programs combine expert personal counseling, along with health
education and support tools, to “coach” participants about healthier lifestyle
choices. Cardium’s professional counselors act as personal health coaches, teaching
participants to plan healthier diets and appropriate exercise routines, monitoring their
compliance with medication prescriptions, and helping them to stop smoking. The counseling
team, including cardiovascular nurses, diabetes nurse educators, registered dietitians,
exercise physiologists, and trained counselors, works with participants’ personal
physicians to develop a customized care plan.
