Cardium Health Receives URAC Disease Management Accreditation

URAC, a leading healthcare accrediting organization, gives Cardium Health
seal of quality for its disease management programs

FARMINGTON, CT (May 2006) – Cardium Health, a leading provider of health improvement and disease management programs to self-insured employers and benefit plan sponsors, announced that it has been awarded Disease Management Accreditation from URAC, a Washington, DC-based healthcare accrediting organization that establishes quality standards for the healthcare industry.

Cardium Health received accreditation for all programs for which it applied including coronary artery disease (CAD), heart failure (CHF) and diabetes. URAC’s Disease Management Accreditation Program standards support evidence-based practice, collaborative relationships with providers, consumer education and shared-decision making.
