Tobacco and smoking cessation primary prevention program. For
people who use tobacco – smoking or chew – quitting is a difficult
challenge. More than 25% of U.S. adults smoke. And while 70% (46 million) of smokers
want to quit, only 2.5% succeed in kicking the habit each year without support.
The Cardium Health tobacco cessation program is an effective solution to help employers
reduce health care, disability and lost productivity costs related to employees
with a tobacco dependency.
The focus of the Cardium Health tobacco cessation program is to help individuals
change high-risk behaviors. We understand the psychology of change and apply the
stages of change theory to match appropriate interventions with each participant’s
state of readiness. That’s why our program starts by evaluating each eligible
candidate’s readiness to change, and we enroll only those who are ready to
make a commitment to quit. By providing intensive counseling and support at the right
time, we catch participants when they are most likely to embrace change. This in
conjunction with nicotine replacement products and medication support* leads to the
highest possible success rate.

Identification and enrollment are based on the Cardium Health blended opt-in / opt-out
model where candidates can self-refer into the program or be identified through health
risk assessments and case management referrals. Because effective employee communications
and program promotion are central to the program’s success, Cardium Health can
work with employers to develop articles for employee newsletters, worksite posters,
paycheck stuffers, and other materials to aggressively communicate the program and
encourage employees to enroll.
Once a participant is enrolled, Cardium Health counselors are skilled at assessing
the participant’s readiness to change. Our behavioral
health specialists provide
motivation through the initial interview process as well as throughout ongoing sessions.
While counselors encourage and motivate participants to make significant behavioral
change, goal setting is also realistic, attainable, and designed to suit each individual
participant. This approach results in maximum enrollment and retention in the program,
high satisfaction, and demonstrated positive outcomes. Additionally, once a participant
quits smoking, continued follow-up and counseling support is provided to prevent relapse.
Employers can expect to achieve a 30 percent “quit rate” for their actively
enrolled employee populations. The program is paced and goals are set according to
individual needs, using guidelines established by the Agency for Health Care Policy
and Research.
* Use of nicotine replacement, Zyban or other therapies is
