Weight management services, an integral component of disease
management programs. Most people
with weight problems know from experience that diets don’t work. Gimmicks, deprivation,
and false promises leave people disillusioned and often no thinner. Cardium Health
takes a different approach. With an understanding of the central role weight management
plays in overall health, our multi-disciplinary clinicians help participants achieve
weight loss goals as an integral part of the overall counseling process.
Weight Management services are included as part of our integrated,
multi-disciplinary
approach.
For example, participants in the Cardium Health back
pain or diabetes programs may also
be overweight or obese. Because weight is a contributing factor to their primary health
issue, we address this as a component of their care plan.
Our registered dietitians, behavioral
health specialists, cardiac
registered nurses and exercise
physiologists work together to provide coordinated and comprehensive care.
While one clinician serves as the participant’s primary health coach, other specialty
health coaches are brought in as needed to provide goal-specific consultations.
Our clinicians have many years of experience and a caring attitude. They provide individualized
diet recommendations, assist with creating food diaries, offer menu suggestions, suggest
exercise programs, and lend emotional support. This integrated, personalized approach
is woven into the counseling process for all of Cardium Health disease and prevention management programs.
At Cardium Health, we realize that weight management issues are complex. A weight problem
is not just the result of environmental factors (large portion sizes, little activity),
or heredity (family history), or health (a slow metabolism), or psychological issues
(stress), but often all of these factors combined in varying degrees. Our people-centered
approach treats each person as an individual with individual needs and concerns.
There is no one-size-fits-all solution to helping people with weight management. Rather,
each person must be counseled individually. And, each of the factors that may be contributing
to the weight problem must be explored.
Cardium Health weight management services include telephonic counseling to address medical
issues and psychological concerns as well as educational mailings to provide nutritional
information and ideas for healthy ways to deal with the stresses of every day life.
