With the ultimate goal to guide participants from enrollment to self-management to graduation,
our Real Caring model begins by getting connected,
an identification and enrollment process
that reaches out to all identified eligible candidates. The success of the Cardium
Health approach is proven by our 75% - 80% enrollment rates with 35% - 45% of participants
receiving telephone calls at least once per month.
The next step to Real Caring is working together.
Cardium Health disease management clinicians fill the communication gap between participants
and their doctors. Our primary health coach model is designed to meet people where
they are in the healthcare continuum and help them
move forward to achieve mutually agreed upon goals. Participants receive health and
wellness materials as well as regular coaching calls scheduled to meet an individual
participant’s needs and convenience.
Telephonic counseling is both professional and personal to offer expert advice in a
caring, motivational way. Cardium Health clinicians not only educate but also advocate
for participants by helping them take full advantage of all appropriate health improvement
programs offered by their employer.
