Behavioral health specialists. For
people with diagnosed chronic disease, their conditions often are the source of intense
stress, resulting in anxiety, anger, sadness or depression. This also is true in the
reverse; stress has a major influence on the body’s systems and can contributes
to numerous physical conditions. In essence, having a chronic condition such as heart
disease or diabetes wears on a person’s
emotional wellbeing. At the same time, if an individual’s original diagnosis
is a mental illness or behavioral health issue, there is a high probability that the
person’s physical state will be affected
too.
The Cardium Health multi-disciplinary model includes behavioral health clinicians with
training and experience in the emotional and behavioral aspects of managing life’s
challenges. These clinicians may include registered nurses, tobacco cessation counselors
and others who have the resources and ability to help participants make better lifestyle
choices and positive behavior changes. They serve as both primary health coaches (PHC)
and specialty health coaches (SHC) depending on a participant’s needs. Our goal
is to provide emotional support and encouragement by offering a combination of education,
affirmation, support, advocacy, and care management in order to help participants understand
and manage their conditions.
With a real caring approach, we listen to the participant to develop a mutual understanding
of the issues and challenges. We establish a supportive relationship and celebrate each
accomplishment on a participant’s journey to better health.
