Certified diabetes educators. Certified
diabetes educators, or CDEs serve as either primary health coaches (PHC) or specialty
health coaches depending on each individual participant’s unique needs for better
overall diabetes care. All CDE clinicians have at least 1,000 hours of direct counseling
experience in diabetes education and have successfully passed a rigorous national exam.
Our CDE health coaches provide education and support for participants with diabetes.
Together, the clinician and participant cover a wide range of topics including the importance
of knowing your numbers for HbA1c, blood pressure and cholesterol; carbohydrate counting;
the importance of regular doctor visits; how to manage sick days; and how to prevent
diabetes complications.
CDE health coaches encourage participants to make gradual dietary changes toward a
more healthful diet. They also help participants recognize the symptoms that indicate
when their medications may need adjusting. Referrals to specialty
health coaches such as exercise
physiologists help participants develop exercise plans that will work with their
individual interests and lifestyles.
With a real caring approach, our ultimate goal is to help participants learn how to
live healthy and happy lives with diabetes.
